Introduction

I am a qualified nurse associate. I completed my foundation degree at Teesside University alongside working at The Queen Elizabeth Hospital in Gateshead. I worked there for 2 years whilst I completed my training. Once I qualified in July 2021 I started working in a GP surgery in Sunderland, then in November 2021, I joined North Gosforth PCN.

What I Do

The main goal of my role is to help support practices with the housebound annual chronic disease reviews. I aim to try to ensure patients are supported whilst at home and provide the care that is needed. I work with patients who are housebound.

How I Support Patients

Once I have been made aware of the patient, I will then contact them via telephone and introduce myself. I will then proceed to book them in for a home visit for their chronic disease review.

I can offer support and advice to patients. I am also able to liaise with other services for additional input with the patients care.
