Social Prescribing Link Worker

My name is Libby Molloy. I joined North Gosforth PCN in April 2022, I am a social prescribing link worker, and I work with patients over the age of 65.

Before joining the PCN I worked have worked in the Freeman, RVI and Queen Elizabeth Hospital. I also worked in the community in the east end of Newcastle.

What I Do

I work with patients referred to the service with non-clinical issues affecting their health and wellbeing which could be social isolation, loneliness, housing issues, frailty and more.

How I Support Patients

As each patient is different we offer telephone consultations, home visits or meet in the community, whichever way best suits the patient.

During the first initial contact we find out what matters to the patient, what is affecting them and create a plan together how we can help moving forward.

Because we work as part of a wider team, we can refer to different services or use our vast knowledge of services and groups in the area to link the patient too.

In the meantime, we will always maintain contact and follow up sessions.
